Welcome to the Ellsworth Community School District!

Ellsworth Community School District uses an online registration process through Infinite Campus.

Before you begin, please gather the following information:
Household information: address, phone numbers, and proof of residency (if new to the district)

Parent information: work and cell phone numbers, email addresses
Student information: demographic, health/medication information and birth certificate

Emergency Contact: addresses and phone numbers.
Note: Required fields are marked with a red asterisk, and the district will receive the data exactly as it is entered. Please be careful of spelling, capitalization and punctuation. Dates should be entered as MM/DD/YYYY and phone numbers as xxx-xxx-xxxx.

If you need assistance, please call the High School at (715) 273-3904, the Middle School at (715) 273-3908, or the Elementary School at (715) 273-3912 during business hours or leave a message and a representative will be back in touch with you the next business day.

• We strongly encourage you to complete the Free and Reduced lunch application even if you are unsure if you qualify. The School Dental form is optional.

• High School students will need to schedule an appointment with the Guidance office to schedule classes. This can be done once your enrollment is approved or call 715-273-3904 to ask for the Guidance office or email deissk@ellsworth.k12.wi.us
